This is an example of the Adoption Contract you will be required to sign. Pet Quality S.A.V.E. is adopting this as “Pet Quality” ie. this animal has been examined, wormed, vaccinated and possibly spayed or neutered depending upon age and healthy to our knowledge. S.A.V.E. requires all adopted pets to be seen by a Veterinarian within 2-3 weeks. If you do not have a Veterinarian, S.A.V.E. will recommend one for you. Even though this pet has been treated with worming products, we recommend bringing a fecal sample with you to this visit. Please call us with the results. Behavior/Temperament/Pedigree of Pet S.A.V.E. makes no warranty, expressed or implied, regarding the pet, or the health, condition, age, pedigree, personality or temperament of the pet. We give you this information based on what we know. Even though S.A.V.E. has evaluated this pet's behavior and has tried to make a "match" with you and your family, S.A.V.E. cannot assure you of this pet's future behavior, and will not be responsible for any injuries that may occur from this animal.
Adoption Donation Fees and Terms:
Kittens: up to and including 12 months old $125 (Does not include spay/neuter.) $75 cash deposit for spay/neuter. Once proof is received deposit will be returned.*DEPOSIT IS RETURNED WHEN PROOF IS SENT NO MORE THAN 3 MONTHS AFTER THE ADOPTION DATE* We will provide you with low cost spay/neuter options or you can choose to use your own vet).
Dogs 8 months old and including 6 years old $350 includes spay/neuter These fees may include: Up to Date and Age Appropriate Vaccinations. Parasite treatments. Testing for FIV/FELV (Aids/Leukemia) in Cats and Heartworm for Dogs (over 7 months of age only). Spay/neuter if age appropriate (over 6 months of age).
Illness Procedure If this animal becomes ill within 2 days, you must call S.A.V.E. immediately for instructions on how to proceed and which veterinarians work with S.A.V.E. However if you choose to use your own veterinarian, S.A.V.E. will not be responsible for the medical care/bills. If you refuse treatment, S.A.V.E. reserves to right to reclaim aforesaid animal. After 3 days you are responsible for this pet and its health.
If the animal becomes ill within 2 days, here are your options. 1.The animal must be taken to your vet. If the animal has an untreatable condition,we will take back our animal and refund your adoption fee 2.If you no longer want the pet, or are unable/or unwilling to pay toward the balance above the adoption fee toward the medical bill, S.A.V.E. will reclaim the pet.
S.A.V.E. reserves the right to reclaim this animal under the following conditions *The adopter agrees they may not abandon, give away, gift, sell, transfer to a shelter or otherwise. You shall notify S.A.V.E. immediately at any time you determine you no longer want to or are able to keep the pet. In the event you are for any reason unable to provide a home for this pet it must be returned to S.A.V.E. You must NEVER bring this pet to a Shelter. If this pet has been given away S.A.V.E. has the right to seize the transferred pet when possible as per this contract. *If the animal was adopted under false or misleading information. *If the animal's welfare is in question by either neglect or abuse. *If the animal was not provided medical attention that was required/needed by new owner. *If the aforesaid animal is a cat and not being housed inside at all times or a dog left outside for long periods of time. *If the owner neglected to spayed or neuter the pet at the specified time as stated on this contract. *If this pet is mutilated such as declawed or ears/tail cropped by above owner. *If this pet is used for any other reason other than a companion animal such as barn cat/watch dog etc... SAVE Cannot take back aggressive or animals involved in bite cases. I agree this pet may not be euthanized except in the case of terminal illness, severe injury, or old age accompanied by pain and suffering and with the concurring opinion of a licensed veterinarian. A licensed veterinarian or veterinarian technician in a private clinic or hospital must perform the euthanasia. S.A.V.E. must be informed of a change in the pets health that would incur euthanasia. S.A.V.E. reserves the right to follow up on the progress of this adoption. S.A.V.E. has a lifetime return policy.
